A few years ago, when I was still playing doctor, the FDA released a "Public Health Notification" regarding plasticizers in IV tubing. Back in 2002, when King W sat on the throne, the gummint advised that hospitals avoid DEHP (if possible), particularly for very young males.

I stormed over to the Chair of Pediatrics (not an unusual event) and showed him the memo. He got excited, and talked to the NICU folks, who were already aware, and had made the changes that were financially feasible.

Watch an old war movie. Look at the IV fluids.

Me: I just want to say one word to you - just one word.
Pediatric Chair: Yes, Mike? [exasperated look].
Me: Are you listening?
Pediatric Chair: Yes I am.
Me: 'Glass bottles.'




Oh, yes, glass safety was raised--I keep forgetting about those pesky babies escaping from their isolettes, tossing glass bottle around when the nurse isn't looking.

BPA is in the news--the FDA has raised concerns about bisphenol-A (BPA), used in plastics. Yep, the same BPA declared safe in 2008.

Unlike most potential toxins, BPA (and DEHP) mimic hormones--BPA was specifically designed as a synthetic estrogen and noted as such back in 1936 (Dodds and Lawson, Nature 137: 996).

Hormones work in minute quantities. Very minute.

I will be squeezing last summer's peaches into a food grade plastic bucket today, along with a few pounds of honey, some chlorinated water, and yeast. The peaches will thaw in the same bowl my mom used decades ago, and I will think of her.

Next week I will transfer the bubbling mess into a glass carboy.

And this is where I am supposed to preach moderation and sanity--we all use plastic and chlorinated water--it's safe, why feed the scaremongers and tinfoil hat crowd?

Why indeed....

Because we are deliberately putting a compound designed to act like estrogen into our food!

National Breast Cancer Awareness Month
is sponsored (and controlled) by AstraZeneca, a pharmaceutical company descended from Imperial Chemicals Industries (ICI), the inventors of polyethylene, the same stuff treated with BPA to make it more useful for packaging.

ICI makes tamoxifen, a key drug used for treating breast cancer. ICI also makes huge amounts of organochlorines, associated with breast cancer.

Breast cancer rates for women here have risen from less than 1 in 20 rate before 1940 to a 1 in 8 chance today. Next October, during National Breast Cancer Awareness Month, check out how much of the official NBCAM literature mentions environmental causes of breast cancer.

So, yeah, everybody's doing it, drinking BPA, and look, we're all healthy, really.

It's a testament to the power of propaganda that the plastics industry uses a known hormone mimic to package food, and the onus falls on those who think this might be a bad idea to show the link between BPA and ill effects.




(During my mom's last week of consciousness, I carried her to the bathroom, her bones settling into my arms as easily as her nightgown. She died before her 60th birthday. So, yeah, maybe I'm emotionally invested in this--doesn't change the facts, though.)

Lincoln logs


In the past couple of days I got to play tug-o'-war with a striped bass (he won), watch a loon chase spearing, and listen to the fog horn go on for hours--it is spring in Cape May.

Last night my brother surprised me with a few gifts. I slipped past fifty when I wasn't looking.

Rock'Em Sock'Em Robots. Labyrinth. A 007 spy camera/gun. A 1959 World Almanac. A classic rod hockey game (nobody ever beat me). A Tinkertoy set. Joe Pepitone, Jerry Koosman, and Tug McGraw baseball cards.

None of it expected, none of it deserved, all of it cherished.

This morning I built a cabin out of Lincoln logs. The container has both a girl and a boy (the boy with an odd expression on his face). Some of the pieces have been chewed on by a puppy who has long died of old age.

My Lincoln logs are made of wood. Some fit together better than others. Every single piece came from a tree, and had to be shaped.

I built a small cabin with a window and a door. I forgot to put in a log. I had to take apart a wall, and when I did, it fell down.

No reset button. Just Newtonian physics. A little bit of frustration.

I rebuilt my cabin. A lot of satisfaction.

Plastics are cheap, and easily shaped. (That's why they're called plastics.) Plastikos: moldable. About a century ago we figured out how to mold oil.

Very low doses of bisphenol A, found in many plastics, prevents chromosomes from lining up right in rats. It has been associated with breast cancer. It has been associated with prostate disease. BPA mimics estrogen.

A year or two this made big news because you can find it in baby bottles. But we knew that already. And we did it anyway.

My daughter loves to fish, but will only fish for what she can eat. She lives in New Jersey. This means she can no longer fish for bluefish. Or striped bass. Or eel. Or white perch.

She can eat one fluke a month, and one weakfish. We'll catch our fluke in June, our weakfish in July.

I'm not supposed to eat bluefish more than 6 pounds. I'm already addled. I'm no longer reproducing. I'm eating bluefish.

My grandfather was born in 1898. He could eat all the bluefish he wanted. He lived a long life, and finally died in the 1990's. He worked until a a couple of weeks before he died.

He never drank water from a Nalgene bottle, and if he ever had the opportunity to drink from Nalgene, water would not have been his first choice.

Plastics were not invented until he was 11 years old.

Toys were made out of wood because that was what was available. Wood is still available. Plastic is cheaper.

Imagine if the Soviet Union sold us baby bottles that harmed out children. Don't worry, it's Dow, GE Plastics, and Sunoco, not the Commies.

BPA was developed in the 1930s, specically as a syntheitc estrogen. Now the States make over 2 billion poinds a year of this stuff.

An estrogen mimic acts like estrogen. Wowzers, stop the presses!

I am a science teacher--hundreds of children have passed through my classes, and I bet not one of them worries about BPA.

Something is wrong.

Lincoln logs did not contain BPA in the 1960s--they were made of wood. In the 1970's, plastic replaced the wood because, of course, plastic is cheaper, and in our culture cheaper trumps just about everything.

Lincoln logs are wood again. The fish are still not safe, but at least a few people are aware of the problem. So this summer my daughter and I will catch a fluke or two, then maybe play Skeeball to replace fishing.

Should either of my children choose to have children, they will know what wood feels like.

A Topologist's View of the Body

Perchlorate is an an interesting compound. It's found in air bags, some lubricants, mortars, and rocket fuel. It's also found in human breast milk.

Perchlorate is present in virtually all milk samples, the average concentration in breast milk is five times higher than in dairy milk....The presence of perchlorate in the milk lowers the iodide content and may impair thyroid development in infants. On the basis of limited available data, iodide levels in breast milk may be significantly lower than it was two decades ago. Recommended iodine intake by pregnant and lactating women may need to be revised upward.

Ah, another environmental fountain of words. More sated Westerners, conquerors of the planet, seek to fight the good fight, to "protect" the environment, this soup we live in.

Maybe you're interested because your mother has breast cancer, or maybe you feel a little threatened by terpenes in your water. You're minding your own business, yet now you have to face some messy nonsense in this "environment."

Swallow a long string. Keep feeding the string as it passes through your gullet. Eventually it will pass out your anus. You can tie the two ends, forming a nice loop. You are topologically related to a doughnut.

(Actually, given the holes in your nose leading to the pharynx, the holes in your nasolacrimal ducts leading to your nose, and the assorted other sorts of holes in your body, you are a bit more complex than a doughnut--we're closer to pretzels.)

It's easy enough to see one as a doughnut still separate from one's environment. Keep your mouth closed, your butt tight, and you can maintain a sense of identity.

Indeed.


Think of a cell deep in your body. The only qualification is that it has to be alive. Let your mind meander to 3 centimeters inside your liver capsule, or maybe you prefer thinking about a kidney cell nestled deep in your back.

If it is alive, it respires--sugar combines with oxygen, heat and motion result.

Life.


We think of ourselves as separate from the environment, and that is partially true, at least at macroscopic level. Still, you must join in the global party of life, consuming bits and pieces put together by sunshine, or you cease to be.

The surface area of your skin is about 2 square meters.
Your lungs? About 100 square meters.
Your gut? 300 square meters.

While our Western sensibilities balk at this sort of nonsense, the rest of our body wants to be exposed. 400 square meters of you wants to absorb air and sugars and proteins and water. You have giant sails of mucous membranes specialized in absorbing anything and everything around you.

Meanwhile, you spew off methane and carbon dioxide, urea and heat.

To say you live "in" the environment confuses the issue. Don't let your skin define you.


Go with your gut.

The Passaic River and broken babies


The shower washes the garden mud off my feet; the reddish-brown water looks like blood as it swirls down the drain. Within a few hours, the millions of microbes washed off my feet will be killed by chlorine, and flow as sewage effluent into the Passaic River, the same river made famous by its ability to ignite. Sewage effluent, fortunately, does not burn as easily as the rest of the river.

The effluent flows as clear, odorless, nutrient-filled water (or so the sewage authorities assert). A few years ago, near the tail end of a long drought here on the East Coast, about 90% of the water in the Passaic River flowing past Newark was treated sewage.

I regularly kayak by the discharge pipe of the Livingston Sewage Authority. The pipe rests near the origin of the Passaic River, really just a stream at his point, my paddle often scraping the rocky bottom. Livingston has gobs of money. The effluent coming out of the pipe smells surprisingly fresh, not what one expects from a poop-pipe. Not odorless either.

I know the smell of chlorine. I know the smell of esters. As an erstwhile organoleptologist, I assert that the Livingston Sewage Authority contains (at least) water, chlorine, and a nose-friendly ester. Either that or the wealthy have poop that smells like perfume.

Man-made esters frighten me a bit. They should frighten you, too.

Carp thrive in this part of the river. Painted turtles reproduce, making baby turtles with the right number of body parts--one head, four legs, one tail. Deer drink from the river, and an occasional hawk soars overhead as we paddle upstream. Maybe the discharged effluent is safe.

Still, I fear the estery smell; while I do not wish to wade in buckets of bacteria, the smell of sweet volatile organic compounds makes me edgy. I imagine my plastic oar melting as I near the effluent waterfall.

When I was younger, I loved the smell of organic liquids. I sniffed gasoline, not to get high, but just for the smell. The varied chemicals stored in the garage got sniffed for the sheer joy of the odor.

I remember giant wafers designed to be loaded in a green, translucent cane filled with water, designed to inject a sweet-smelling herbicide directly into dandelions. Groundskeepers had a high rate of leukemia; we just did not know that yet.

The mosquito man would drive his truck through the neighborhood shooting his white, dense cloud of sweetness--we'd hop on our bicycles and get lost in the fog. VOC junkies.


Sperm counts are dropping. More and more boys are born with incompletely formed penises. No one is sure why, but phthalates have been implicated. Di-n-butyl phthalate flows with the effluent of treated sewge. It smells sweetish, like an ester.

Di-n-butyl phthalate makes plastics more pliable--it is a "plasticizer." Male reproductive organs may become more plastic as well.

While "fringe" ecologists have been screaming about endocrine disrupters for years, the plastics industry continues to maintain that there is no hard evidence linking phthalates to human hypospadias.

Still, enough evidence exists to raise the eyebrows of the Food and Drug Agency.
Everyone is exposed to small levels of DEHP [Di(2-ethylhexyl)phthalate] in everyday life. However, some individuals can be exposed to high levels of DEHP through certain medical procedures. DEHP can leach out of plastic medical devices into solutions that come in contact with the plastic. The amount of DEHP that will leach out depends on the temperature, the lipid content of the liquid, and the duration of contact with the plastic. Seriously ill individuals often require more than one of these procedures, thus exposing them to even higher levels of DEHP.

Exposure to DEHP has produced a range of adverse effects in laboratory animals, but of greatest concern are effects on the development of the male reproductive system and production of normal sperm in young animals. We have not received reports of these adverse events in humans, but there have been no studies to rule them out. However, in view of the available animal data, precautions should be taken to limit the exposure of the developing male to DEHP.

I discussed this with the chair of the department of pediatrics. The FDA warning interested him, but fixing it costs money.

Glass is too expensive. Glass breaks, too dangerous on a pediatric hospital floor. A shrug.

I have raised the issue with other doctors. More shrugs. People shrug a lot these days. I shrug a lot. What can we do? A shrug acknowledges a problem as unfixable. Dismissive.

Changes in the sexual morphology of fish exposed to sewage effluent have led some scientists to conjecture that humans also live in a "sea of oestrogens" and that the apparent increases in the incidence of certain reproductive conditions may be due to exposure to chemicals in the environment. The so called Sharpe-Skakkebaek hypothesis offered a possible common cause and toxicological mechanism for abnormalities in men and boys,that is, increased exposure to oestrogen in utero may interfere with the multiplication of fetal Sertoli cells, resulting in hormonally mediated developmental effects and, after puberty, reduced quality of semen. It was postulated that synthetic chemicals in the environment are the prime source of the excessive oestrogenic stimulation, with exposure through food and water being the primary route.

Repairing a hypospadias is not difficult if the child has not yet been circumcised. The estrogen-like effects on a developing fetal brain are not known. Shrug.

Phthalates are less of a concern than shit. Poop smells like, well, poop. Phthalates smell like esters, vaguely reassuring. Refreshing.

Who is going to worry about a river that used to catch on fire anyway? Shrug.

Another friend with breast cancer

A friend has just been diagnosed with a recurrence of breast cancer. She has a young daughter.

While I pretend to teach science in the classroom, quite a few of the young women I teach today will lose a breast (or two) in the next few decades because of environmental factors.

I remember the first breast I saw no longer attached to the body it once helped define. I had seen body parts in various forms before, but this one was fresh. A flap of sallow skin with a wizened nipple defining it, a long trail of fibrous fatty tissue trailing off the slab.

The pathologist, smoking as he dictated, handled the breast like a butcher handles meat about to be weighed, though not as kindly.

Incidences of breast cancer change in populations as people migrate from one area of the world to another, suggesting that environmental factors contribute to this disease. There is a continuing effort at the NIEHS to identify these environmental factors and the role that exposures to specific chemicals could play in this disease.

I shaved my mother's head when her cancer recurred--bony metastases in her skull made the shaving more difficult. She walked like a marionette the weeks before she died. In a radiology reading room, we'd call them "goobers." Goobers on the brain.

Unless it was one of our mothers, our sisters, our daughters, our friends--then they were metastases.

Since 1985, Zeneca Pharmaceuticals has been the sole funder of October's National Breast Cancer Awareness Month (NBCAM). Zeneca has promoted a blame-the-victim strategy to explain away escalating breast cancer rates, which ignores the role of avoidable carcinogens. Zeneca's parent company, Imperial Chemical Industries (ICI), is one of the world's largest manufacturers of petrochemical and chlorinated organic products -- including the plastic ingredient, vinyl chloride -- which has been directly linked to breast cancer, and the pesticide Acetochlor.

In addition, Zeneca is the sole manufacturer of Tamoxifen, the world's top-selling cancer drug used for breast cancer. In return for funding the "awareness" campaign, ICI/Zeneca has control and veto power over every poster, pamphlet and commercial produced by NBCAM. "A decade-old multi-million dollar deal between National Breast Cancer Awareness Month sponsors and Imperial Chemical Industries (ICI) has produced reckless misinformation on breast cancer," said Dr. Epstein.

The media focuses on the strength of cancer survivors, and I have seen tremendously strong women live and die graciously through months and years of chemotherapy and radiation and surgery. The magazines will show glossy pictures of proud women, and these things matter, of course. Avon will sell "Kiss Goodbye to Breast Cancer Lipsticks," Mars, Inc., will sell you pink and white M&M's, and General Electric will sell you a Senographe 2000D mammographer.
They do not show a mother cowering in her bathroom, her bald head bare, blood all over the toilet from a nosebleed that will not stop, her teen-age son standing awkwardly, bravely holding her head.

They do not show the vomiting, the pain, the fear. They do not show a mother with her arm in a machine trying to squish out the fluid building up from lymphedema. They do not show the bony protuberances on a skull, the smell of dying cells.

They do not show a child wiping her mother clean because she is too proud to use a bedpan and too weak to use a toilet.

In 1991, these were the 6 most common carcinogens found in breast milk. The news has gotten worse since then. We are at the top of the food chain--toxins accumulate.

It has been known that breastfeeding reduces your chance of getting breast cancer. The longer you breastfeed your babies, the lower the risk. This has been attributed to hormonal changes related to breastfeeding--breastfeeding women cycle less, and had less exposure to estrogen.

There has been speculation (and it is only speculation), that breastfeeding may help reduce the chemical pollutant load on the mother. Guess who gets the chemicals?

The lifetime risk of a woman developing breast cancer was just less than 10% in the 1970's, or 1 in 10; it is now 13.4%, or almost 1 in 7 (NCI, 2005). In the 1940's, the risk was 1 in 22. Breast cancer is the leading cause of death in women 34 to 54 years of age.

Janet Jackson flashes a breast, and our Federal government rushes in to redefine obscene. Certain words and phrases will cost lots of money.

Here's an obscene phrase that won't cost anything--in fact, every October you hear it dozens of times:

Early Detection is the Best Protection.


This makes no sense--once detected, you already have it. The best protection is prevention which, admittedly, would require massive, radical changes in the way we live.

The NBCAM folks got wise--they now say "Early Detection Saves Lives"--if you go to their website, they pretend that this is what they have always said.

So it must be true.
